Punch Treatment For Acne Scar Removal

Ajith Laxman

Punch treatment is one of surgical methods employed in the treatment of acne scars. Severe scars that cannot be treated by natural skin rejuvenation and other topical creams can be treated by means of punch treatment methods. This method is widely used in the treatment of ice pick scars.

The following are the punch treatment methods used to treat acne scars:

1. Punch Excision: This method is used to correct deep skin scars like ice pick scars and boxcar scars. It consists of a tool with a variable head that comes in contact with the skin. Depending on the size and surface area of the scar on the skin that needs to be removed, the head can be adjusted and made to come in contact with the scar surface. The scar can then be excised using the tool and the outer layers can be sutured together. The sutured skin layer forms a scar but it fades away quickly. This method is usually carried out in the presence of a local anaesthesia.

2. Punch Excision with Skin Graft Replacement: This method is used when the skin excised covers a large surface area. In such cases, suturing of the skin surfaces may not be possible and hence, skin grafting could be considered, where the removed scarred skin could be replaced with a portion of skin taken from behind the ear. A visible difference could be noticeable immediately after surgery, although it fades out over time. Skin resurfacing methods like chemical peel and microdermabrasion could be used in conjuction with this method.

3. Punch Elevation: Deep scars can be corrected with this method. Also, punch elevation reduces the risk of further scarring like in the other methods. The scarred skin surface's edges are incised and the base of the scar is left intact. The excised base is then evelated to the surface of the skin using suitable glue like dermabond.
